- CHS Athletic Participation Forms - DRAGONFLY
All Cambridge High School sports will be utilizing the DRAGONFLY health management system to manage our Athletic Participation Forms process in 2020-2021.
Please use the link below to find out how to register your player with the Dragonfly system to complete all health and participation forms necessary for Cambridge sports. This forms packet must be completed, submitted and approved by the CHS athletic trainers prior to athlete participation in any preseason and seasonal track & field workouts and training.
